Forms and Resources

Massanetta4

We are required to have a medical release and information form on every youth that attends any events. We fill these out each fall and they are good for the school year.

Youth Information and Medical Release Form

BMPC has implemented a new Child Safety Policy that helps ensure that all our children and youth are cared for and nurtured by screened and trained adults. Please check it out and become familiar with our policies. We have had no incidents here at BMPC, but we are aware of the real world around us and we are working to keep BMPC a safe place for us all. Our policy includes:
• careful screening and training for all youth advisors who regularly work with our youth.
• never allowing one youth and one adult to be in a room alone. We believe in private conversations and appropriate intimate relationships, but they need to be done in a public, visible space.
• This means that our youth advisors can NOT give youth a ride home – that puts one youth and one adult in a car alone.
